I can not find a definitive document on how and where to setup approval hierarchy. I don't see anywhere that i tell GP who is a manager other than the PO req workflow. I can't find any documentation on how to even setup groups. I want to say if this group of users creates a PO Req this is the approval process it needs to go through. I could create a workflow for each user but that seems crazy and ridiculous to maintain.

You can simply create multiple workflows and yet activate only one, that is an important option when you have an existing active workflow, and willing to create another completely different workflow, you can keep one WF active and yet build another one in parallel.
As for the documentation, there is an important book by Ian Grieve on this subject GP Workflow 2.0

one other question while i have you...... I have not figured out why if you can only have one workflow active at a time why would you need more than one? If i create a workflow i need to be running all the time otherwise the approval process would not work correctly. Right?? what am I missing??

This is where the WF 2.0 starts to get a little bit clunky... when you need approval from people that are not in the standard hierarchy of the AD tree... You have no choice to create custom approvals in those cases. Try to keep them to a minimum as they are a pain to maintain.
There is an "advanced" mode in WF 2.0 to allow you more criteria than the default ones available, and you may have to grab to them in some cases.
To enable the advanced mode, you have to navigate into :
Administration >> Setup >> Company >> Workflow >>Workflow Maintenance >> Workflow step >> select the Action is required when the following condition option and then choose the expansion button. In the Editor form, check the box "Display Extended Field List" to get more than the basic 5-6 condition fields...

I did see your post. My issue was more my lack of experience with AD. As a GP consultant typically AD management is done by the client. I now understand where the Manager Designation is in AD. I am still working on understanding exactly how it works so i can direct the client as to what i need them to do. My current question is if i designation a user as someone's direct manager for purchasing workflow purposes what if that is not the case for say a timecard approval.

Normally the workflow approval process for WF 2.0 is driven by the Active Directory (AD) hierarchy... but you can chose to create your own in GP if that doesn't fit your need.
