My form does not capture the contacts

Hi

I have a problem with a marketing form, it was working and capturing leads but suddenly submissions stoped and show failure.

What could be the reason for that?

    Hi Ewelina,

    the exception says there is no telephone1.

    Can you make sure you add one?
