Script syntax error trying to load MFA login

A client recently started getting a script error when trying to send EFT Remittances via email.  It is coming up at the point where the MFA login would usually appear and we can see the frame of that window, but the script error is preventing the user from logging in.  This was working as recently as a week ago.  The client can't identify anything that may have changed in their environment.
