We have got an issue with all of your Lookup Values, when you click search, its only pulling a few records and when you type in data to search, it doesnt do anything
Any ideas as to why it would just stop working

Do you have any lookup filtering configured or any JavaScript for that lookup field?
Have you checked if this is happening to multiple find columns or only with one?
Maybe you can add the same field multiple times on the form, you would drop the same lookup filed on a separate section or tab check if it still does the same.
Or create a new form and drop the lookup in there and see how it behaves.
Also,Could you please go to Settings -> Customizations -> Entity -> Account -> Quick Find View, check the Find Columns and Filter Criteria.
