i have checked the custom button exists by using ribbon workbench.

There are three types on Forms where buttons can be put.
Homepage – Tabs that are displayed in the main CRM 2011 application screens.
Form – Tabs displayed when a specific record form is opened.
SubGrid – Tabs shown when a list view is selected on a specific record form.
I think your button is on Form and you are looking at Main Grid. You have to create a button in HomePage to display it there.
Regarding New button being disabled. This button is controlled by Create Permission so check if your user has create permission. Also See if this button is not hidden.
Also I observed that , in the grid details where all the records are shown this is not visible but on a single record it is present . this is from the other crm instance.
This is because, you have created the Button in the Form not in the Home page.
but in my crm organization it's not visible.
Please check, to which instance of CRM you are using Ribbon workbench.
also i am not able to insert a new record and new button is also invisible.
Please check the Display and Enable rule of the New button
Also I observed that , in the grid details where all the records are shown this is not visible but on a single record it is present . this is from the other crm instance.

Please see if the button is not hidden on Ribbon Workbench.. If Not, check if there are any display Rules on the button.
