payroll distribution breakdown report incorrect

Hello all. When my client prints the Payroll Distribution Breakdown History Report, it displays incorrectly for only one employee. It somehow is printing two distributions lines for the same account. For instance, one line is for a deduction amount and the line under it with the same account number has an amount that is totally different (some 8x the amount). I looked in the UPR30401 table and the account index has duplicates. What could be causing this?
