Build a function that creates a record in a custom entity with the below fields whenever an appointment for an account is completed.
• Account Name – from Account
• Account Type – from Account
• Account Country – from Account
• Appointment End Date – from Appointment

i need to create this for activity entity but this is not shown in workflow .
what i can do for that?

You can definitely do so.
I just created a Case Entity Record, when Appointment is marked as Completed.

I gave 2 condition to create a record.

1. Status Reason should be "Completed"

2. Regarding field in appointment shall be of Entity Type Account.

Note: I used Case for refrence, same way you can create record for Custom Entity.
