I would like to design a new questionnaire format as below table design. Can anyone advice how to make in standard questionnaire setup.

Hi,

you can look at tutorial forms / tables in AOT.

i.e. AOT\Forms\tutorial_Form_Table or AOT\Forms\tutorial_Form_DisplayOptions.

Those may help you to achieve your goal
