We randomly have an issue with duplicate records being created, both in custom entities and in Opportunities/Orders. I have a theory that this could be happening if a user is trying to save their record at the same that I am publishing changes.
Does my theory hold any water? Or is there a way to trace how the duplicates were created?

Hi Barbara,
When you import a solution, Dynamics CRM automatically unpublish all the Duplicate Detection rules if the entity is included in the solution.
As a part of solution deployment, you need to always ensure that after import, all the required duplicate detection rules are in published state (if not, publish again..this may take some time depending upon the size of the entity records). Note that untill the state change to published, the rules were not in active state and can allow duplicates.
In addition to this, the same may happen with auditing.

When publishing a solution, CRM will disable duplicate detection rules.. Quite often, there will be a plugin that will re-activate the rules (so, maybe, you have a plugin like that. But there is still a moment when the rules are not activated)
