Hi all. I write this post in order to help others who struggle with an issue on why you cannot add the 365 App for Outlook to a specific user (after having approved and tested the mailbox).
Normally you can use this check list in order to set correct security setting roles for the user https://docs.microsoft.com/en-us/dynamics365/customer-engagement/outlook-app/v8/deploy-dynamics-365-app-for-outlook
And normally this should not be a problem to standard security roles (I was using the 'salesperson' role). But I found that after a Online 9.0 upgrade, I couldn't activate the 365 App for Outlook. Using Xrmtoolbox to compare security roles I found that 'prvUseOfficeApps' was missing. According to linked article above you set the privilege this way:
In the Privacy Related Privileges section, verify that Use Dynamics 365 App for Outlook is set to Organization. If not, click Use Dynamics 365 App for Outlook
But in CRM this option is not showing! - see here:
I remember the option was visible in the past (and it's not even visible on a brand new 9.0 customer I have launched recently). So until Microsoft has fixed this (fell free to run a support case - I don't want to use the extensive time on it) you can try to add an extra security role to the user - I added the 'Marketingmedarbejder' (Marketin Employee) and it allowed me to add the 365 App for Outlook to the user.

Here's a short update and a fix:
UPDATE: I'm running 3 support cases now on this (i.e. 118073118696096) - it's a general bug in 9.2 and I was told it should get fixed in build 0002.1429 (expected release date is end august).
WORKAROUND: use XRMtoolbox, download the plugin "Role Updater" and set the priviledge manually.
Again thanks for helping with input Ben, but now I tried again using the old Sales Person default Security Role, and then added this Security Role to the 365 Outlook App in the new App designer. But this is not sufficient. The user is still missing the prvUseOfficeApps priviledge :(

I notice the same instructions (in bullet 5), but the "My App" section gives extra information, and I'll try this tomorrow (the "My App" section has not been added to the sitemap by the upgrade so I'll need to fix this first). I'll get back with a post on succes or not
The version 9 app is very different from the previous version (8) app as is the backend infrastructure of Dynamics 365 as a whole..
I don't know why you are looking at the old version of the documentation as the latest version is at docs.microsoft.com/.../deploy-dynamics-365-app-for-outlook
