Just wondering if anyone has a link or a reference to a sample real-world dynamics ax 2012 project explaining from requirements to developing/testing and deploying for learning purpose.
Any link or open source project would also be fine as it would it give us an idea.

Every implementation project is unique, and partners are using their own practices and implementation methodologies to deliver a project. There are no implementation samples provided, the only possible way to acquire this knowledge if you get affiliated with a successful partner and learn the nooks and crannies there first-handed on a new project.
As a framework you have the Microsoft SureStep methodology that is a generic project delivery tool with some typical document samples, you could have a look.
Larger projects are typically using Agile code releases with a Prince2 project delivery methodology (or their own version of that).
You may find books as well about how you should conduct an implementation for AX.
