I am having this same issue. I have followed all the steps listed here and in the guides linked and others as well. I cannot get the Entity Form to be displayed on my webpage. I've exhausted my troubleshooting options and am now hopelessly posting onto the forums.

Can you please check the page template you are using, is it showing as Rewrite / web template , if it is showing as rewrite and having an aspx info there then try to add entity form, else use web template instead of form.

If you want to display an entity form, it needs to know what record in CRM to view. Are you navigating to this entity form page from an entity list page or just a weblink? If from an entity list, you need to specify the query string so the portal knows what to show and the query parameter needs to match. If just from a weblink, you will need to add some logic to the page to know what record to show.
