Hi Teddy,
When you created the case template ( I assume it is word template?) did you add the related Note fields (in your case Created On)? IF so, then you should see this field in the word document and you should be able to add it.
First, when you create the template hit Select Entity
Then select Note and hit Download
Then under Incident_Annotation fields, right click on createdon and add it to the document.
Please make sure that Word doesn't auto upper case the first letter of it as this happened to me and because of it, it didn't work properly.
