dependent option sets sdk code. getting an error on using the code "reference error: sdk not defined at eval code"

I have created two option sets called state and city. and have added xml web resource creating options I need.and then followed following steps in the following link https://docs.microsoft.com/en-us/previous-versions/dynamicscrm-2015/developers-guide/gg594433(v=crm.7) . now on load I am getting error sdk not defined at eval code. anyone please help me solve this problem  .

    Hi,

    Did you complete step #3 -Add the sample_SDK.DependentOptionSetSample.js Script web resource to the JScript libraries available for the form?

    If yes, try refreshing your form or try different browser.

         should I add anything else other than xml webresource and javascript webresource which s given in thsteps of the above link to the crm.
