Can someone direct me as to where I can find the PSTL tool Account Modifier/Combiner. I can see PSTL in the navigation pane but clicking on it does nothing.

It may not be installed or corrupted. Login with "sa" and try. If not installed already, clicking on the PSTL setup menu will install it. Otherwise, you may completely uninstall PSTL and reinstall.
