MSCRMAddin Event ID 5958

An error occurred setting up a Microsoft CRM toolbar or button. Restart Microsoft Outlook and try again. HR=0xef604005. Context=. Function=CAddin::HrSetupToolbarOnSingleExplorer. Line=232.

That is the error I receive in the event viewer. Best I can tell there is nothing wrong with the crm or the users outlook.

I've googled and googled and found nothing.
