Can not add webresource (.png, .jpg) to entity form- webresource does not show in form after insert screen

I'm attempting to configure a custom action type Plugin, to be fired by a  button, following walk through here: http://blog.customereffective.com/blog/2012/01/xrm-2011-crm-2011-style-buttons.html

Upon completing the Insert > Web Resource dialogs (Gerneral and Formatting tabs) and clicking "OK" on the dialog, nothing happen- no control appears on the form. I've tried multiple permutation- pre-selecting various controls on the form etc... what gives?

    Also, I have also made sure to Publish the Web Resource (.png) and that does not seem to be issue.

    Have you saved and published the changes to the form itself?

    Bingo !

    I tried it in IE 10 and that made the difference- I now can see the "plc_placeholderButton" form control within the selected Tab
