Can anyone tell me if the 'Add New Activity' button in the 'Open Activity Associated View' has ever been available for closed cases?

At least not in 2013 - that I just checked.
For a quick workaround, if you don't want new activities to be associated with closed cases, create a sync workflow for each type of activities, add a condition to verify if the case is closed, and stop the workflow as cancelled - you users will be getting "business process error" with whatever message you'll define in the workflow step properties.
