Falling out of business flow (BFP)

Hello

I have a simple business flow that creates an invoice out of an opportunity that has 5 stages. It used to work perfect, but then at some point in testing things, I found that when I go from step 4 (entity: opportunity) to step 5 (entity: invoice) I lose the business flow process header completely :O

When I click "Next" stage to the right, a menu appears asking to whether select a linked invoice or create a new one (+ sign). When I click the + button, I used to have the new invoice form opening but with some of the opportunity's information filled it.

Now the BFP header disappears.

Things I did:

- Deactivate/Activate process

- Check security roles, everyone has access to it...

Anything else ? Using CRM 2016.

    This is weird. By default, the roles selected for a BFP are System Administrator and System Customizer. If I say everyone has access to this BFP, this is when the header disappears when switching from invoice to opportunity. If I go back and re-select only System Administrator and System Customizer roles as having access to it, the header still disappears going from opportunity to invoice...

    If I save a copy of the workflow and use the copy, it'll work again. But if I delete the workflow that was used to create the copy, it stops working... header disappears.
