W2 State, Employee Summary, and Employee State Tax Summary Incorrect

I have an employee that was paid twice in 2013, once in Q1 and once in Q2. For simplicity each amount was $5,000 in taxable wages for $10,000 total. Fed, SS, MC wages are all correct on the W2 and employee summary screens. The W2 state wage is showing $15,000 or one extra check. The Employee State Tax Summary also shows 3 payments one being in Q3. The Employee Summary screen shows the last check being from 8/15/2013. If I sum the taxable wages from UPR30300 I get $10,000 and the last check date is from Q2 in UPR30300 and UPR30100 for this employee.

Where is GP pulling this "phantom" check from? I've tried reconciling the employee and it did not change the Employee State Tax Summary or Employee Summary screens.

It looks like a transaction was entered for 8/15 but then deleted but it is not updated in the above screens or the year-end W2.

Any ideas?

    Ok I ended up putting a trace on the database and found out that the [zDP_UPR30301SS_1] SP is getting called when I view the Emp. State Tax Summary screen. It looks like the UPR30301 table is not correct and was not properly updated for whatever reason.

    Any ideas on why this did not get updated? I inherited this GP install after this was done so I'm not exactly sure how the extra transaction was posted and removed. It does look like it was a BEG BAL manual check adjustment.
