Hello All
i want to add a field which give facility enter date and time using calendar as give below image .that field i will use in form

You just need to create a new field in your entity having data type as date and time,
Navigate to Settings -> Customization->Customize the system->Select your entity->Fields->New
Place your field on your entity form, save and publish. Now when you will see this field, and will provide you calendar control.

Create a Data Type field in an entity say Contacts set the format to Data and Time. Place it on form, save and publish the form.

Simply by creating a new Data Field, and setting its format to Date and Time.
Then drag it to your form and publish it :)
