Can you please help me to resolve above error message coming while saving visit report in CRM?

Could you provide more information on the background of this problem?
For example, what version of your D365? How did you create the Visit Report? and some more.
Hi, I am using Microsoft Dynamics 365 Version 1612 (9.0.9.4) (DB 9.0.9.4) on-premises. I tried to create Visit report from Workplace ->Customer and Installed base -> companies. Then I opened required company and from company there was a visit report view -> clicked on +ADD NEW VISIT REPORT button and tried to create new visit report and entered basic information and once I clicked on SAVE button, I got this error.
Did you write any custom plugins or activity in message create at "Visit Report Entity"
Hello, No I didn't write any customer plugins or activity. I just opened any random customer from Workplace -> customer and installed base -> companies and in related section of that company, I have option "visit report" and once I click on it, I can see visit report associated view and in that view, I have a button called " + ADD NEW VISIT REPORT" and I clicked on this button and new form opened and I filled in basic information and hit the SAVE button and got this error message.
can you try "Add associated Visit report" instead of "New visit report"
I don't have that option "Add associated Visit Report" only I can see +ADD NEW VISIT REPORT button

Is there any javascript methods working in save event at Visit report form??
if no, can you trace Event viewer of your server and see the errors
