I am using Dynamics 365 and following instructions exactly as they appear here @ https://nishantrana.me/2017/03/13/applying-theme-in-portal-in-dynamics-365/ but the new theme will not apply after several hours? This was my first load of a file that I pulled from www.bootswatch.com Any ideas?

Facing the same issue here and After adding the child File .css still not getting the layout to be applied. Does theme.css influence any of this? should it be removed?
The picture behind the CONTOSO text can be changed/removed in the Web Files section of your portal. There will most likely be multiple .jpg and .png files in there, but one of the files should be the picture you're looking for. If you go into the notes section of a Web File record, there should be an image file which you can either remove or change.

Do you know how to get rid of the image that comes at the top of the Customer Portal?

How about pressing CTRL + F5 to force the download of the new bootstrap file?
Thank you for your response, I did restart the portal at least four times. I even removed the bootstrap file and reloaded a new one. Any other ideas? I did notice that my browser would not download CSS file but instead wanted to open it but Microsoft Edge actually open it in Notepad
