Custom Page when Clicking on Lookup

So I have made a custom page and have it linked to when I create a new record from a sub-grid, the list grid, or when opening it from the list grid it launches perfectly.
I followed this YouTube tutorial: https://www.youtube.com/watch?v=99uU5gGN15w
 
The issue I am facing is that when I click the record when it is in a Lookup field, or attempt to create new from the lookup field it will launch the original Model Driven default form.
 
Any assistance would greatly be appreciated. =)

    It is non customisable. So youncan try to hide by modifying the form XML.
