I want to have table in the form. How can I achieve this.

I'm not sure what you mean by table.
You can include a subgrid, which is like a table.
it will show multiple records based on a view and filter criteria of your choice.

Subgrids work the other way round
name address acc no pn no
Main
secondary
The reason for this is if you had 10 records then it would be hard to show all fields across the top
if you want to show records in the format you want then you would need to create a custom web page
