Missing Entity Icon for custom entities within Interactive Service Hub

Hi all,

I have a problem in which my custom entities are not displaying their entity icons in Interactive Service Hub, no matter how many times I change and re-publish them from the customization section.. The lookup search will always show a missing icon, and the src of the image goes to a 404 not found. Does the Interactive Service Hub have a different icon or way of setting it?

    This seems to be a problem that was affecting an older version of Interactive Service Hub in Dynamics CRM 2016 on-premise. Once I migrated the solution to Dynamics 365 online, the problem disappeared.
