This error is coming when i add custom entity form , not on system entity. Any guesses , what could be the issue??

Not sure if this is still an issue for you, but in case others encounter it, I figured a link to MS's solution might help: docs.microsoft.com/.../entity-forms
Probably your entity form is too complex and you have to choose single tab (field: Tab Name) to be used for portal presentation.

The best way to work with the issues is to see the actual log from the portal as recommended by Aric Levin
But most likely you have a problem with custom entity form (some custom controls or more probably some control appears several times). You can create a simple form for that entity adding the only subset of the fields to it and try with this form only.
At least it is the quickest way to confirm that problem actually exists with the custom entity form.
