Skipping check #'s

For some reason, the system has been skipping check #'s.  I can't figure out how to fix this issue.  Of course I want them to go in sequential order.  Thoughts?

    Thank you, we have a multi-user environment and we have had this same scenario occurring and was not able to come up with a valid reason.
