Customizing the Voice of the Customer Survey Response form

We are wanting our users to be able to add notes and comments easily to our customer's survey responses. Some thoughts we've had:

1) use JavaScript to present the survey question and results in a "multiple lines of text" field, and then the user can directly add their comments under the customer's answers. This isn't ideal though.

2) Is there a way to directly edit the web resource that is presenting the survey results in the table form? If we could add a couple rows of "comments" fields that our users could edit - that would likely solve the problem.

Any other thoughts on some possible approaches?

    Hmm...I am also having trouble displaying the survey response web resource on another form...any thoughts on why this could be?
