1. Create a NEW CUSTOM ENTITY in AX 2012 R3 ( step by step )
2. ADD CUSTOM FIELD in to Standard ENTITY. ( step by step )

How about trying with the links I shared. They are quite good and I'm sure you can follow them. Perhaps you can even document your process and share it here? :)
Anyway nobody has done exactly what you require in your project so you need to digest existing info and apply it in your unique situation. As far as I remember you have posted here for some years already so you can definetely do it!
And both docs are exactly what you asked = step by step so I'm quite sure you can't get much more detailed instructions.

I don't think that the community can help you with some project related documents that are taken and shared from an implementation project. That would be against most regulations partners/consultants get into when they work on projects.
Personally, I find the information that Nikolaos shared already very detailed.
If you need more information then I would suggest that you contact the authors of the blog post / docs sites for more information or guidance on that.
Would that be a good way forward for you?
