From the description on the link you provided, its function is to display the attached image of the note on other related entity form, not add it to note form.
As you said, the note form cannot be customized, so you can download to view it in note form, the pcf carousel control can't work in note form directly.
For example, show note image in the contact form after adding one note with picture to it.
1.Go Settings > Solutions to import file that just downloaded.
2.Go Settings > Customizations > Customize the system > Entities > Contact > Fields.
Create a new Single Line of Text field and add it to the form, then click the field to open ‘Field Properties’ dialog and move to ‘Control’ tab.
3.Add ‘Carousel Control’ and configure it as document.
4.Test.
if the contact not add any notes or note's attachment isn't picture, the filed will show empty.
