Confused as to why Live it not showing same as Sandbox

Hi,

I have a dilemma which I need help with.

This started with a previous post of mine

Business Unit Specific New Contact Form - Dynamics 365 General Forum Community Forum

I carried out bit more testing in the Sandbox before manually transferring it over to the Live environment.

I documented the process and ensure I customised the Onload for the Form, using the Java script

I've doubled, tripled checked the Business Unit, Team, Managed Role, Security Groups the are the same. N.B There is no data in Live for the Business Unit yet but I was expecting the New Form at least to be present??

I know this is a long shot but would anyone know what I'm missing as I think I've looked at this for too long

To add to the confusion when I view these using my admin credentials the screens are reversed??????
