We have upgraded GP 10 to GP 2013 R2 and we use extender table on Customer master for adding extra details on shipto addresses. I assume once GP upgraded the EXT00101 & EXT00103 is blanked out and data gone into new table EXT01100, EXT01101 & EXT01103. But I couldn't find is whether this is the exact changes happens while upgrading. Does anyone have experience with this, please guide me changes happening on tables and fields of the above.

Click on the Extender 2013 Table structure, then go to Page 110 - it will show you all the mappings between old and new extender.
The changes that you are seeing is perfectly normal. Extender backend structure has to be redone for several reasons. Specifically the way the forms and windows are now being defined.
You may want to check their blog that details all that you need to understand.

the existing extender customer still can upgrade to latest version. there is no problem in the upgrade and all works fine, but just wanted to know the details about table change.

Have you contacted eOne support regarding this? They took Extender back from Microsoft at the end of 2012.
