Customer Voice Mobile Version distorted

Hello community,
 
Has anyone else encountered the issue of the mobile version of a customer voice survey page being partly not responsive?
It affects the header: as soon as a logo is selected the header doesn't adjust to the width of the screen anymore.
 
Any suggestions?
