Is this possible to get the list of all fields including form name which each user added through personalization settings. If yes, then how can we achieve this ?

just click personalize, you can see the personalized fields as the below with ++ sign

Actually we want to get the list of all forms and its fields which were added through personalize, instead to going on each form separately. This is because many users don't know at which form they added fields through personalize. So instead of going on each form they want the overall list.

The table SysLastValue contains all personalizations. You can check those records initially on form level. A container field in this table has the details as packed object.

Hope you are fine. I initially checked this table but found no meaningful data in it. The field name as Value in this table contains varbinary type data e.g "0x07FD010500000001D4000000..." So how can i convert it and make it meaningful.
If you go to the forms/classes where the personalizations are managed, you can try to reverse engineer how this field can be "decoded".
Can you please share which classes are used to manages this personalization settings?
Possibly you will get your answer quicker by searching yourself. I don't know by heart and don't have the time to check it for you.
