Incoming document automatically attaching to new Purchase Invoice

Hi, I have a new issue where if I go to create a new purchase invoice, a previously posted incoming document attaches itself to the new record straight away. 
 
It seems to be cycling through documents that were posted earlier in the day, so I'm getting a different one each time.  I have no idea why this is happening, is there a background setting/cache that might need clearing?
 
Any help greatly appreciated.

    Hi, this is a bit strange, does your environment include any customization? You can uninstall and try again. If there is no problem, it means it is caused by the customization.
     
    In addition, this may happen if you use an OCR service. But your situation seems to be different.
