Lead entered on marketing form is saved multiple times in Dynamics CRM

Hi all,

I have a Marketing form and linked it to a web page on an external site.

The form is display as designed but when I click the Submit button two things happen:

  1. I get the error "An error occurred during your submission." See my previous question: https://community.dynamics.com/365/marketing/f/dynamics-365-for-marketing-forum/383754/error-when-submitting-marketing-form-on-external-web-page
  2. The lead records is saved multiple times with the same timestamp. See screenshot

I am not sure if these issues are related, hence I am posting them as two separate questions

Installed Version:
Microsoft Dynamics 365
2020 release wave 1 enabled
Server version: 9.1.0000.14440
Client version: 1.4.274-2002.3
Dynamics 365 Marketing 1.35.10059.2039
