Hi Evan,
I have faced same issue and fixed by changing Default tab to Notes( same solution provided by Shaminderpal Singh) but keep in mind if you switch tab to ASSISTANT then to NOTE will give the same issue ( I am not sure the issue is only for me). In this situation just refresh page to add notes.

Hi Evan,
I experienced the same,it' a bug induced in 9.0 latest version with relationship insight.
You can resolve it by opening form editor->Notes Text and changing the default tab to notes.

Could you try to create file from portal using admin credentials.
Even I faced the same issue, not sure how but resolved now.

Hi Evan,
Please make sure, you are having appropriate permission to attach note to webfile. Someone has not accidentally remove your role.
Also try the same in IE browser.
