Why would the audit history area of a record show information for 1 record and then 2 other records show nothing? They are coming from the same entity, so auditing is on and enabled.

Did you enable the all fields for this entity? I mean that if you only enable for some specific fields, and you didn't change those fields on 2 other records, it would show like your description.
