Customer Voice - new survey title not displaying on browser tab

I started a survey by copying another survey.  I changed the title on the survey.  Although the new title displays on the survey, the old title displays on the browser tab when the survey is opened to be completed as an end user. 
 
Note that the survey title was changed over a month ago and the browser cache cleared, so that is not the issue.  I have tried in multiple browsers and in incognito with the same result.  The old survey title always appears on the browser tab.
 
See attached screen shot. Please advise how to make the title that appears on the browser tab to match the title of the survey.  I have checked all settings and this forum and do not see anything related to this.  Hoping someone can help.

    • Open your survey in Customer Voice.
    • Go to the "Send" tab.
    • Click "Preview".
    • On the preview page, check if the old title shows in the tab — it confirms the issue.
    • Go back to the "Design" tab.
    • Look for the Survey name at the very top (this may still be the old name).
    • Click the survey name (at the top of the page), and rename it — this updates the actual form/page name (not just the title shown on the survey).
    • Save changes.

    Hello Muhammed - Both the form name and the title are displaying correctly.  See additional screen shot showing both the form name and the title.  It is only the tab that is displaying the old title.

    I discovered that there was text in the hidden Header.  It is what is in the Header, not the Title or the Form Name that controls what displays on the browser tab.
