Error: Unable to parse the OrganizationServiceFault while debugging plugin in CRM 2015

Hi,

I wrote a plugin and registered it in plugin registration tool, when I am trying to debug it I get below error, not sure what is wrong, it doesn't let me debug the plugin or hit the break point in VS. CRM is onpremise, version 2015.
