Approve leave option missing from manager self-service

Would anybody please advice.

The CEO doesn't have an approval button at his Self-Service to approve a leave request from his direct reports.

The workflow stops with fault: 

Stopped (error): X++ Exception: A worker with a personnel number of was not found.
at SysWorkflowHierarchyProvider-resolve
SysWorkflowHierarchyProvider-resolveHierarchy

The worker and CEO are set up as workers and enabled users. 

Only CEO experiences this problem.

An ideas how to resolve it would be appreciated.

    Hi JanaW,

    Please confirm if the worker associated with this user CEO is still active.

    You can check the employment history of this person.

    Please try it and update with us.

    Yes. Worker is active.

    I have noticed that his position (CEO) is flagged as not in hierarchy. He is a CEO therefore on top of the reporting structure.

    Could that be the source of problems?

    Hi JanaW,

    Yes. This is the problem.

    The workflow will pass the request to the manager, which is the upper layer, so the CEO needs to be in the hierarchy.

    How can I add the CEO position in the position hierarchy. By default his 'reports to' field is empty as he is at the top of the hierarchy.

    Thank you. I have succesfully added CEO's position to the hierarchy.

    However, the CEO still cannot see the 'approve' option on his self-service. Any ideas please?

    Can you show me the item message in the workflow history?

    Does the CEO have the suitable privilege?

    Hi QianQW,

    Status Pending. No error messages (since we corrected his position in hierarchy with your help, thank you).

    The CEO has the same manager privileges as other managers (they all can approve leave without any issues).

    The requests sits in the workflow history as pending. And the CEO can see it in his Team self-service. But there is no button/option he can click to approve it.

    Hi JanaW,

    Cancel the request workflow and create a new one to check it.  Make sure the position hierarchy changed is active for this workflow.

    In addition, show me the screenshot of the work item assigned if it still doesn't work.
