Maybe it helps others around here: Recently stumbled over a few issues reported where ModelDrivenFormIntegration is missing in the Canvas App editor - even though embedded in Model Driven App. Same here :-(
Suggestion apparently is to always use the "Classic" mode and "Customize".
As alternative it SEEMS to be possible to - if in Canvas editor - just append
&is-hosted=true
to the URL and the control is displayed.
