Based on the below screenshot I would like to know how can I populate the fields Lead contact Name and Lead Contact Organization from the Concatenated Lead Name?

Are you talking about getting the data from the Contact and Organization entities? If that is the case, you can use WebApi to retrieve the Contact and Account.
