Error when adding custom entity to the Unified Interface App

Whenever I try to add the custom entity to the unified interface app, I would get the below error when I try to open the app.
If I add the system entity (e.g. user, team, and etc) there is no error. What can I do to resolve the issue?
I am using on-premise v9.
Something has gone wrong. Check technical details for more details.

Error_Message_0x8005f210 Inner Error Message: The value passed for ConditionOperator.In is empty

    Hello,

    In your case it can be a problem with the environment itself.

    There are other errors reported in the community, with the same error message, but regarding different operations:

    If your environment is Dynamics 365 Online, I would suggest you to open a case with Microsoft, so they can help troubleshooting that error message.

    I did some research and I was able to fix this issue by creating the quick create form for entities.
