Reports duplicate listings on reports

Hi All,

On various reports I run (custom and OOB) I often get two listings for the same account. What is causing this? Is it something I'm doing or can prevent? In the example below, just simple report to get names and numbers, but my test account (of which there is only 1) is listed twice.

Also, important to note, the two listings do not always match up. Further down this same report we have two listings for Customer "X"

-one listing has all the columns filled out

-one listing has none of the columns filled out.
