Scanning Qr Code and then the data of qr code must go in there perspective field in microsoft dynamics 2012

I made the QR code ssrs report and when i scan it with my mobile it show all fields right but my concern is how can i extract the data from that generated code and fill the fields with that particular data.

I take 5 fields 

a.Itemid

b.Location

c.On hand Inventory 

d.Description

e.Latest Recieving Date

I just want all the fields fill with data automatically when i scan my generated qr code.
