In such an organizational structure, would two separate Business Central instances be required? Would these be in separate databases, in like kind?

All depends on FDA requirements here (as far as I know FDA mainly focus on labeling, tracing, UDI, etc). If we are talking about HIPPA than answer can change.
But first you need to check if FDA requirements are in Database level or on user/operational level. If they are on Database level then if Microsoft compliance is acceptable and BC Admin is compliance to manage both companies then you can have two companies under one instances. But if you need to have two BC Admin to manage each DB then you need two separate instances.
For user/operational level you can have both companies under one DB but you need to carefully plan users access. The only challenge is if you need customization that only for one company. You can check and see if extension can be written to only impact one company and not the other one.
