Dynamics 365 Segments not Auto Refreshing

Problem Statement:
We have created segments in dynamics 365 RTM. The segments target audience is contact and the segments Refresh Rate is Auto Refresh.
Further we have created Journeys and associated this segments to those journeys. The journeys Frequency is set to "A one-time journey where newly added audience members can start any time". 
Issue:
After creation everything works perfectly fine for few days and then all of a sudden the segments stop auto refreshing. 
Even though the new contacts are created the segments does not refresh and hence the journey does not trigger.
 
Currently this issue is for 5 of segments that we have created. Issue occurred 3 times at least and every time we have to recreate the segments/Journeys.
 
Any solution/help would be really appreciated.

    A couple of things to look at:
    • Are you up against quota limits for marketing contacts or monthly interactions? I'm assuming based on your recreated journey's working, the answer is no.
    • Is something happening on the contact record that prevents them from being added to the segment? Possibly a new workflow has been added that is causing the new contacts to be filtered.
