Form Captures stop working

Hi,

We have issues with our form captures. 

After implementation they work fine, submissions are delivered within Dynamics. 

Only a day later, submissions will not be delivered anymore within Dynamics. When we rescan the form they will do again but not for a long time. And also the campaign automation is not work

Does anyone have the same experience and a solution how to solve?

    Is there no record of submission in Marketing form > Insights > Submissions either?
